According to teasers for Oshi no Ko 143, Ruby is hoping that Aqua will refer to her as Sarina chan, the way he did when they were together in the past.

There really shouldn’t be any responses because, jokes aside, this outcome was painfully evident from the beginning. However, I do not enjoy it. Having an adult and a child become siblings through some sort of reincarnation plot makes absolutely no sense whatsoever.
Remove the “past lives” and reincarnation elements, and the story could continue just as it does now—with Aqua and Ruby being their true selves from the start, as childhood friends who shared an idol and are now collaborating to uncover her killer in Oshi no Ko 143 spoilers.
Sarina Tendouji may even tell Aqua how she feels, given that she went to such lengths to claim that their relationship would consist solely of intimacy.
Because of this, or maybe even after production wraps, Aqua will have to deal with a lot of internal moral and emotional conflict, even if he feels responsible as a writer.
What Happened in Chapter 142?
Kana, Miyako, and Mem-Cho begin Chapter 142 of Oshi no Ko, “Responsibility,” by talking about how they found the kissing sequence between Aqua and Ruby to be both exciting and unsettling. They figure out that Ruby’s eagerness is due to her brother’s fixation.
